Help to identify possibly medieval Polish silver coin

Hi, I have this possibly medieval Polish silver coin. Its diameter is just under 20 mm. Could I please have some help in identifying this coin? I would like to know its denomination, date, ruler and issuer. Also if it is already on Numista please direct me to its page. Thanks

1. The crossbar is between the letters "O" and "M" / the crossbar is under the letters "O" and "M"
2. The placement of the date.
3. The figure at the bottom.
